Transcript Request Form

When you are finished with your exchange studies you can receive your official transcript of records by submitting the transcript request form below.

Exchange students completing their studies this semester will receive their Official Transcript of Records approximately three weeks after the official end date of the semester. To receive the form you need to request it individually.

The official end date of the spring semester 2018 is June 10, 2018.

The Official Transcript of Records will be sent by postal mail to the address you register in the Transcript Request Form. Please make sure that your address is correct before sending the form.

Please fill out the below form to order your offical transcript for you and/or your university. Do not send the request form until all your grades are reported into Ladok and you can view them in your Student Portal account! We only accept transcript requests when all grades are reported.
